It's great that you got it working but it looks like your
commit 659cbbc13f41 caused a lot of unexpected formatting
changes.

I committed 61a5d3fc151 after rebuilding the documentation with your
SGML updates and tidying with "HTML Tidy for FreeBSD version 5.8.0"
which again caused lots of formatting changes.

Which tidy version do you use?
